Thank you for visiting Members Health Plan NJ. As you may be aware, The Affiliated Physicians and Employers Master Trust (APEMT) d/b/a Members Health Plan NJ (MHPNJ) filed Chapter 11 Bankruptcy on 5/24/2021 and began the orderly wind down of the plan. APEMT/MHPNJ (the “Plan”) ceased writing new business and as of December 31, 2021 is no longer covering members.
If you need to contact the Plan regarding a past due balance or the assessment that is due, effective February 1, 2022, you must discuss this matter with the Plan’s legal counsel, SM Law PC. They can be reached at (908) 572-7275.
If you need to contact the Plan regarding an existing payment plan arrangement, please email your inquiry to MEWAbilling@concordmgt.com. Please include your account number and the nature of your inquiry.
If you need to contact the Plan regarding 1094 or 1095 forms, member issues, or other issues please email your inquiry to MEWAenrollment@concordmgt.com.
